The Yard – East Flatbush Family Enrichment Center (FEC) operated by Rising Ground is one of 30 FECs launched by the Administration for Children’s Services in New York City.

Participation is voluntary, free, and open to all community members. At the centers, families and children can connect with neighbors, volunteer their time, participate in various events and activities, and access resources and supports they need to thrive.

The Yard began providing community offerings last year including Financial Literacy, Computer Science & Coding, and Vision Board Workshops, a Summer Youth Program, and a monthly Women’s Virtual Circle. Events have included a Community Baby Shower, Back-to-School Resource Fair, Thanksgiving Turkey & Food Giveaway, a Holiday Toy Giveaway, and more. Community members are invited to take part in bi-monthly virtual Town Hall meetings to express their ideas about what they want to see at the FEC.

The Yard opened its physical location at 3521 Church Avenue in Spring 2025. The Center includes a community kitchen, business center, children's library, food pantry, community wardrobe, and meeting space for members of the East Flatbush community.

Connecting Resources to the Community and the Community to Resources

The Yard – East Flatbush Family Enrichment Center (FEC) operated by Rising Ground is one of 30 FECs launched by the Administration for Children’s Services in New York City.

We provide evidence-based primary medical care for children from birth through young adulthood who live with foster parents, kinship families, in residential or in Close-To-Home placements.

      Giving a voice to infants and toddlers who have been exposed to trauma.

      Seen & Heard gives voice to infants and toddlers who have been exposed to trauma, allowing caregivers to intervene before trauma can alter a child’s chances at long-term success.

      We help children from birth to age 5 and 11-months-old to heal wounds left by trauma, violence, child abuse, neglect, domestic violence, gang, and community violence.

      The Circle of Security Parenting Group

      Join our group for caregivers of children age six and under

      English & Spanish sessions are available.

            Soundview Family Resource Center 4Being a new parent is a time of wonder and excitement. You are filled with joy at each new development of your child — her first smile, his first steps, the first time she says, “Mama!” But it is also a time of questions and challenges. “How do I deal with this new behavior?” “My child didn’t eat much at all today. Should I be worried?” Nearly 40% of families in the Bronx live at or below the poverty level, which is an added hurdle.

            We help families meet these challenges at the bilingual Family Resource Center in the Soundview neighborhood of the Bronx, where all services are free. We bring parents and caregivers together to build a community of support and to provide a safety net for families. Targeted to families with children under the age of 5, the Center equips parents and caregivers with knowledge, stability, support and access to resources so they can help children develop to their full potential.

            Building Bonds with Children and Parents Alike

            The Center hosts a wide range of free family and community activities — playgroups, movie afternoons, festivals, and field trips — so parents and caregivers have the opportunity to come together with others in a warm, comfortable environment. Parents and children bond while meeting neighbors, building a sense of community in a neighborhood with limited outlets and where families, particularly those with limited English-language skills, might otherwise feel isolated.

            Parents and caregivers, along with their children, join us weekly for free arts, crafts, readings and other activities. Our staff model how parents can interact with children to promote positive social, emotional and intellectual development.

            In addition to activities for families, parents and caregivers can share and learn from each other through workshops and other activities, such as a moms nights out, a parenting support group and more.

            The Parenting Journey

            For parents and caregivers who need more intensive help, including those involved in Family Court, we offer a 12-week, evidence-based curriculum called The Parenting Journey. In addition to helping parents develop a greater understanding of child development, the program helps uncover patterns of behavior and shifts the focus away from the child to the welfare of the family as a whole. This curriculum encourages positive parenting and gives parents the tools they need to build united, strong families.

            The Parenting Journey

            For parents and caregivers who need more intensive help, including those involved in Family Court, we offer a 12-week, evidence-based curriculum called The Parenting Journey. In addition to helping parents develop a greater understanding of child development, the program helps uncover patterns of behavior and shifts the focus away from the child to the welfare of the family as a whole. This curriculum encourages positive parenting and gives parents the tools they need to build united, strong families.

After traveling as much as 1,500 miles to reach safety in the U.S., we provide them with the emotional and medical support needed by unaccompanied children, who have no one to advocate for them.

              The next step in their journey takes them to a short-term shelter, such as our Passage of Hope Unaccompanied Migrant Child program. After their grueling journey, we help these children be children again. In a nurturing environment, we provide what they have been missing — enough food, clothing, and the opportunity to play and learn.

              We provide clinical, therapeutic, educational and case management services while working to link them with family members or other sponsors in the United States. Our goal is to provide these courageous children, who have experienced incredible trauma, with the support, nurturing and care that they need to move forward with their lives and to thrive.

              Rising Ground’s portfolio of services also includes providing home studies to determine the appropriateness of sponsors and post-release services to ensure that youth are transitioning well into their new lives in this country.

              Preparing for the Next Step

              As part of our program, we help reunite the children with the family members or other sponsor they are searching for. We make sure that their new homes will be safe and nurturing by conducting home studies of those who have been waiting for them to arrive. While waiting for a permanent place to live, the children learn about American culture, they learn English, and they attend school. At Passage of Hope, children move from danger to safety, from fear to hope.

              In 2024, Rising Ground launched the From Hope to Home program, which consists of expanded home studies to further assess the safety of homes and potential sponsors of children, as well as post release services, which assist the sponsor/family with the supportive services they need to ensure the child's physical and mental well-being once they are living in the community.

                • Get support if you are raising youth with mental health and/or substance use challenges. 
                • Find information and resources available that meet your child/youth and family’s needs.

                Crisis Intervention

                • Get professional help at home or in the community when a child or youth is distressed and can’t be managed by family, friends and other supports.
                • Get support and help with using crisis plans to de-escalate the crisis and prevent or reduce future crises.family’s needs.

                Rehabilitation Services

                • Learn to incorporate therapy goals into everyday life and receive extra support managing medication.
                • Build relationships and communicate better with family, friends, and others.
                • Learn self-care, and use coping skills to manage emotions.

                Youth Peer Support & Training

                • Work with peers with similar experience to develop skills to manage health challenges and be independent.
                • Make connections to natural supports and resources.
                • Get help to prepare for the transition to the adult health system.

                  Care Management Services are the helping hand for families that need it most. We help parents and caregivers manage the many challenges they face each day – from finding the right specialist for their child, to ensuring transportation for appointments, to coordinating medication management with schools, to name a few. Most importantly, we assist families with negotiating the many moving parts of multiple systems to make sure the systems work together in a comprehensive manner with shared goals tailored to each child’s needs. Through this holistic approach that includes the whole family and brings multiple service providers to the table, children with various health and mental health needs can thrive and grow.

Thanks to this support, children demonstrate improved health outcomes and a reduction in hospitalizations – and families experience less stress.

                  Care Coordinators continually communicate and share information with families/caregivers from providers in ways that are meaningful to them, sometimes acting as translators. Our services also ensure care during transitions from inpatient to other settings and promote ongoing medical and behavioral care to prevent crisis. Ultimately, we focus on securing the long term success for every child and his/her family.

                  Who Qualifies for Care Management Services?

                  Care Management services are available to Medicaid-eligible children in New York State with one or more qualifying conditions. All children within a family who meet these criteria are eligible, and children can qualify even if they are enrolled in other services, such as mental health, prevention, or foster care.

                  To qualify, a child must be eligible for Medicaid and have one single qualifying condition or two or more chronic medical conditions.

                  Single qualifying conditions include:

                  (Serious Emotional Disturbance is often indicated by a child’s inability to learn or develop or maintain relationships with others, while Complex Trauma is common among youth who have been victim of or witness to abuse, neglect, violence in the home or the community, or who have been displaced from their homes or had parent figures removed from their lives)
