Healthcare systems are often complex, confusing, and difficult to access, especially for those dealing with illness or who face social barriers to care owing to social determinants such as language, immigration status, or a lack of resources. Many individuals struggle to understand insurance coverage, medical bills, provider networks, and the steps required to receive care. Without guidance, people may delay treatment, face unexpected costs, or miss out on services they are entitled to.
By assisting patients in understanding their options, completing paperwork, and communicating with providers and insurers, Rising Ground’s Healthcare Navigation team helps ensure that people receive timely care and make informed decisions about their health. This support not only reduces financial and emotional stress but also improves health outcomes and promotes a more equitable healthcare system for everyone.
Our services are provided in two ways:
Community Health Advocates
Assisting individuals with any health insurance-related inquiries, including enrollment, renewals, navigating medical billing, and finding local providers.

Social Care Network
A New York State initiative specifically for Medicaid members. We help eligible individuals access enhanced services such as food delivery, cooking supplies, and rent/utility assistance.
Eligible populations include:
- Pregnant and postpartum women
- High risk children under 18-years-old
- Individuals enrolled in a NYS Health Home
- Individuals with intellectual and/or developmental disabilities
- Medicaid high utilizers
- Individuals with a serious mental illness
- Members with a substance use disorder
- Post-release criminal justice involved population with chronic conditions
